The NDIS “Participant Statement Workbook” is a handy tool to assist Participants and their Carers in thinking about and documenting their needs and goals in preparation for discussion with the NDIA (National Disability Insurance Agency). The purpose of a one-page profile is to provide a summary of person-centred information that people in a person’s life can use to either get to know them quickly, or ensure that they are providing consistent support in the way that the person wants. By considering what is important to and for a person, and what good support looks like, one page profiles illuminate actions that make a real difference in a person’s life. This animate explores the ways that a one page profile can be created with people who do not use words to communicate.
